Access Website: This Website offers Access to Eligibility Support Services for Health and Nutrition! This website is a quick and easy way for people who live in Wisconsin to find out if they might be able to get:
Help with buying food
Low- or no-cost health care for families
Help with buying prescription drugs
This website will take you about 15 minutes to use. We'll ask you to tell us about the people in your home, the money you get from a job or other places, your housing costs and a few other bills. What you tell us will stay private and secure. When you're finished, we'll tell you if you might be able to get help through programs like Food Stamps, BadgerCare, Medicaid and SeniorCare. Please keep in mind that this website is just a test to see if you might be able to get benefits. You'll have to apply for these programs to get a final decision about benefits, but we'll let you know how to do that.

Brown County Human Services at the Sophie Beaumont Building, 111 N. Jefferson St, 2nd floor offers the following services and/or programs:

Food Share Program : Individuals and families may be eligible for assistance with food costs through the Food Share program. The goal of the Food Share program is to make it easier for persons to have a healthy, adequate diet. For regular households, the food share benefit amount is based on household size, an income limit and a $2,000 asset limit. The Wisconsin Quest Card (Wisconsin's Electronic BenefitsTransfer Program) allows Food Share clients access their benefits through point-of-sale (POS) terminals. Some Food Share recipients, as a condition of eligibility for Food Share, must participate in the Food Share Employment and Training Program.

Medical Assistance Medicaid remains an entitlement. This means that anyone who wants can apply for Medicaid and have their eligibility determined by a public employee. Medicaid is a state/federal health assistance program which provide certain low income persons help in paying medical bills. You must meet certain criteria for this program.

Health Start Medical Assistance This is a form of Medicaid that is offered to children under age 6 and pregnant women in households with income above the Regular Medical Assistance limits (up to 185% of the federal Poverty Level). Children Age 6 or older, born after September 30, 1983 (currently up to age 14) in households with incomes above the Regular Medical Assistance limits (up to 100% of the federal poverty level) may also be eligible. The Healthy Start program does not have asset limits.

BadgerCare Medical Assistance This is a form of Medicaid that is offered to uninsured , low income families with income limits at 185% of FPL for applicants and up to 200% for recipients. Participants with income over 150% FPL will be required to pay a small premium. There is no asset limit.

Family Planning Waiver This is a Medicaid Program that started 1/1/2003 for women between 15 and 44 years of age. It covers selected family planning services and supplies. This income limit is 185% of the Federal Poverty Level and there is no asset test.

Child Care Assistance Provides a child care subsidy to W-2 employment position participants, families who require child care to obtain or retain employment, and teen parents in school. All eligible families will be funded. Parents must use regulated child care providers and will receive an authorization for reimbursement or voucher. Parents are required to make co-payments based on their income and family size, the number of children is subsidized care and the type of child care provider rather than the cost of the child care. Brown County Human Services provides this service through a sub-contract with Forward Services Corporation.

Low Income Home Energy Assistance Program (LIHEAP) Households may receive assistance through this program to help pay heating bills during the heating season (October 1 through May 15). Recipients of LIHEAP receive a one-time benefit for each heating season. The program pays a portion of the heating bills to the fuel supplier. The amount of assistance depends on the heating fuel, household size and household income. This program is available through Integrated Community Services at 201 W. Walnut St, Green Bay, WI. Forward Service Corporaton - W-2 Program & Emergency Assistance Program

Wisconsin Works (W-2) W-2 replaces monthly cash benefits with services that support work; help in finding and keeping a job, child care, health care, employment-related cash loans, transportation assistance and more. For those who are not job-ready, W-2 works with employers and community organizations to make subsidized jobs, work experience through community service jobs, and other paid work-readiness placements available.

Emergency Assistance This is a program designed to meet the immediate needs of households facing an emergency due to: Fire; Flood; Natural Disaster; Energy Crisis or Homelessness.

Forward Service Corporation's Supportive Housing Program

The purpose of the Supportive Housing Program is to assist homeless families and/or individuals with rental assistance. Forward Service will also provide the family and/or individual with job skills, education aggressive case management and supportive services to become self-reliant. Forward Service Corporation will use aggressive case management to meet with families and/or individuals on a bi-weekly basis and develop an Individual Service/Employability Plan. The plan will list in detail the steps needed to take to find and maintain employment or other sources of income (SSI, etc) to help with education in renting and budgeting for the future.

NEW Community Shelter Inc

The NEW Community Shelter, Inc. is located at 301 Mather St., Green Bay, WI 54303, phone 920-437-3766 offers Emergency Shelter including individualized supportive services and length of stay plans, case management; Community Meal Program for both residents of the shelter and those in our community in need plus basic tools and resources to clients including personal hygiene items laundry facilities, vouchers of clothing, transportation assistance, employment related tools, and education and life skills services and training. Many on-site programs and partnerships with outside agencies support this effort.

Wisconsin Managed Care Enrollment Specialist Project

The Wisconsin Managed Care Enrollment Specialist Project promotes health programs to assist low-income families with appropriate referrals and face-to-face support. The Enrollment Specialist provides public outreach and education about Medicaid Managed Care to community-based agencies, advocacy groups, or any group with an interest in the Medicaid population. One of the reasons we are able to carry out this goal is our solid knowledge and experience with health care programs for low-income families and Medicaid Managed Care. The Medicaid Managed Care Enrollment Specialist Project provides the following services: · Education related to managed care and the HMO enrollment process. · Assist clients with Medicaid Managed Care enrollment, HMO changes, or exemptions from Medicaid Managed Care enrollment when appropriate. · Education related to client rights, responsibilities, and the complaints or grievance process. · Education related to available health programs for low-income individuals and families.
